State Vaccination Requirements

Getting vaccinated is an important part of students' school readiness and keeps children from catching and spreading diseases that can make them sick and potentially disrupt learning. Colorado law requires students who attend school to be vaccinated against many of the diseases vaccines can prevent unless a Certificate of Exemption is filed.

Required Vaccines for K-12

  • Diphtheria, Tetanus, and Pertussis (DTaP)
  • Polio (IPV)
  • Hepatitis B (HepB)
  • Measles, Mumps, & Rubella (MMR)
  • Varicella (Chickenpox)
  • Tetanus, diphtheria, & pertussis (Tdap)

Required Vaccines for Preschool

  • Diphtheria, Tetanus, and Pertussis (DTaP)
  • Polio (IPV)
  • Hepatitis B (HepB)
  • Measles, Mumps, & Rubella (MMR)
  • Varicella (Chickenpox)
  • Haemophilus influenzae B (Hib)
  • Pneumococcal (PCV)

Information on exemptions can be found at https://cdphe.colorado.gov/vaccine-exemptions.

  • Parents/guardians seeking non-medical (religious or personal belief) exemptions for children in preschool must submit non-medical exemption forms every school year.
  • Medical exemptions only need to be submitted once and require the signature of your child's healthcare provider (e.g. physician, advanced practice nurse, or physician assistant).
